Neurilemmoma of the Vagal Nerve, Report of a Case

Abstract
|
|
|
Nerve sheath tumor is a well-defined lestion even though it has received many different labels: Neurilemmoma (Stout), Perineurial fibroblastoma(Penfield), Palisaded neurinoma(Geschickter), Neruofibroma(Ewing), Schwannoma(Masson), and so forth. It is a common tumo of the spinal nerve root, peripherl nerve and the acoustic nerve, and in its pure form may be considered benign.
Neurilemmonmas of cranial nerve origin are uncommon, and especially those of vagal nerve origin have been reported only sporadically in serveral instances. But, as far as we know no case of neurilemmoma arising from the vagal nervel has been reported in the Korean medical literatures yet.
A case is being reported because we encountered a new syndrome of benign sheath tumor of cranial nerve which has not previously been described in the other cases.
The features common to all cases roported consisted of: 1) bulging, firm submucosal tumor mass presenting in the postero lateral pharyngeal wall, causing dysphegia and, later, interference with he airway, 2) pulsation of the umor mass due to medial displacement of the intermal carotid artery, and 3) interference with function of the nerve involved. But this patient, although the left vagal nerve was involved at the bifurcation level of the common carotid artery, did show none of the above signs, but pressented coughing whenever the tumor mass was compressed or palpated.
The tumor mass was successfully enucleated, remaining he vagal nerve fiber intact, and discharged without complication. On follow-up one week later on compression of the left lateral neck where the tumor had been situated previously. Coughing persisted.
A case of neurilemmoma of the left vagal nerve is reported with a brief review of the litertures.
